The GSBS Administrative Form includes: Approval of Credit (other credit, substitution, transfer and waiver); Approval to Defer Admissions, Class Schedule Change, Grade Change, Graduate Assistant Certification, Graduation Term Change, Leave of Absence (personal), Program / Plan Change, Program Withdrawal, and Request to Import Application Record from CAS.
